How to set up CompanyOn e-consent templates

Module Overview

The Consent Templates module allows clinics and practitioners to create, customize, and manage consent forms used for client care and clinic workflows inside CompanyOn.

Consent templates can be used for:
• Treatment consent forms
• Privacy agreements
• Intake consents
• Virtual care consents
• Cancellation policies
• Clinic policies and agreements

How to Access Consent Templates

  1. Log into CompanyOn

  2. Open the Admin menu

  3. Navigate to the Templates Module

  4. The Templates dashboard will open


How Consent Templates Work

Consent templates work similarly to chart templates.

There are two main areas:
• The Form Builder
• The Template Library


How the Form Builder Works

The Builder is a drag-and-drop editor.

On the right side:
• You will see the available components

On the left side:
• You will see the blank template area

To create a consent template:

  1. Drag components into the template

  2. Organize the sections

  3. Edit the information inside each component

  4. Save the template

How the Template Library Works

The Template Library contains pre-made consent forms that can be previewed and added into your Builder area.

Once added:
• You can fully customize them
• You can edit all text
• You can remove sections
• You can add new components

  1. Open the Template Library

  2. Preview the available templates

  3. Select the template you want

  4. Add it to the Builder

  5. Customize the template if needed

  6. Save the template

• The completed consent form is automatically reflected in the client timeline
