How to Activate and Send Online Forms / Consent Forms

CompanyOn allows you to send any type of form electronically to clients before or after care delivery, including consent forms, intake forms, assessments, and custom documentation templates. The process is the same for all form types.

How to Send a Form to a Client

  1. In the left sidebar, click the people icon to open the Clients module.

  2. Click on the client's name to open their timeline.

  3. Click the "Online Forms" icon at the top right of the timeline.

  4. Select the form you want to send.

  5. Under "Method", select how you want to send the form:

    • Email — sends the form to the client's email address

    • SMS — sends the form to the client's phone number

  6. Click "Send".

Note: Once the client completes and submits the form, it will automatically appear in their timeline and you will be notified.
