How to Print a Consent Form

  1. Go to the Admin menu at the top right of your screen.

  2. Select Templates.

  1. Find the consent form you would like to print.

  1. Click the PDF icon beside the form.

Once the PDF version opens, use your browser’s print option to print the document.

The printed PDF can be brought to the appointment for the client to review and sign.
