How to Document Care from an Appointment

  1. Open the checked-in appointment

  2. Click the Form Icon at the top of the appointment window

  3. Select the chart template if applicable

  4. Complete the documentation

  5. Save the chart entry


What Happens Next

• The chart entry becomes part of the client timeline
• Documentation is automatically attached to the appointment
• The client record updates immediately


Important Notes

• If a chart template was pre-selected through the Service settings, it will automatically appear when opening the Form Icon
• Multiple chart entries can be added if needed
• Documentation can be edited later depending on clinic workflows and permissions
