How to Create Chart Templates
Log into CompanyOn
Open the Admin menu
Navigate to the Templates Module
The Templates dashboard will open

• The Form Builder
• The Template Library
What Is the Form Builder?
The Builder is where you create and customize templates.
The Builder works as a drag-and-drop system.
On the right side:
• You will see the available components or fields
On the left side:
• You will see a blank template area
To create a template:
Drag components into the blank template
Organize the sections
Click on each component to customize the information inside it

Steps to Use a Template from the Library
Open the Template Library
Preview the available templates
Select the template you want to use
Add it to your Form Builder by clicking on the “+” icon
What Happens Next
Once the template is added to the Builder:
• You can fully customize it
• You can edit each component
• You can remove sections
• You can add new components
• You can modify all text and workflows
The process is the same as building a template from scratch.

Once templates are created or customized in the Builder area, they become available for documentation workflows.
Templates can be used:
• Through the client timeline
• Directly from appointments by adding them via the services’ pre-selected templates field
How to Add Templates To Services
Templates can be linked directly to services.
When linked:
• Practitioners can document directly from appointments
• The template becomes available through the Form Icon inside the appointment window
Steps to Link a Template to a Service
Open the Services module
Select the service
Navigate to Pre- Select Chart Entry field
Select the chart template
Save the service settings
What Happens Next
• The template becomes automatically available during appointments for that service
• Documentation workflows become faster and more standardized

Open the Clients Module and click on the desired clientTimeline
Select Add Entry in the Timeline
Select the chart template
Complete the documentation
Save the entry
